Registration

To register to the Conference, please fill out the form below, and arrange for your payment. The deadline for registration is April 25, 2003.

The registration fee depends on whether you are a permanent faculty member, a post-doctoral fellow or research associate in a temporary position, or a graduate or undergraduate student. The fee structure is as follows (all amounts are in Canadian dollars):

Faculty member	$125.00
Postdoctoral fellow	$75.00
Student	$50.00

The registration fee does not include the banquet. If you wish to attend the banquet, please indicate so in the registration form below, and add $35.00 to your payment.

Unless formal permission has been obtained from the local organizer (Eric Poisson), the registration fee must be paid in full before May 2, 2003. The following modes of payment are possible: credit card (VISA or MasterCard) and money order.

To arrange for your payment, please fill the payment form and fax it to the number appearing on the form. The information contained on the form will be handled with the utmost confidentiality.
